SERDC Health Services is accredited by the Canadian Accreditation Council to provide exceptional 2nd level services to eight or more First Nations communities in Manitoba. We are community-driven and are looking for a motivated individual who is willing to learn and make a difference in First Nations Health. The successful candidate will be a Nurse, Dietitian, Athletic Therapist or Physiotherapist that will work with SERDC communities and 3 independent communities to assist in their implementation of the Aboriginal Diabetes Initiative (ADI). The Tribal Diabetes Coordinator (TDC) is responsible for assisting community health workers in the coordination of community-based Type 2 diabetes prevention and management programs and services. This position provides capacity building opportunities, supports, and advises communities, related to addressing diabetes in all SERDC communities. The program goal is to reduce Type 2 diabetes among First Nations people by supporting health promotion and primary prevention activities and services to communities.


  • Oversee all aspects of program development and delivery
  • Support ADI program objectives, activities and projects at the community level and work with SERDC communities to ensure that objectives are met
  • Prepare reports and proposals for funders and liaise with referring sources
  • Develop culturally appropriate strategies, programs and activities for promoting Type 2 diabetes awareness, prevention and management at community level
  • Assist community-based staff to assist in empowering individuals and groups to make healthy choices (nutrition, physical activity) to prevent and manage Type 2 diabetes
  • Develop and deliver culturally appropriate training and capacity building opportunities to meet identified training needs for community-based staff
  • Provide consultation and support to First Nations and community staff on pertinent developments in diabetes and related fields such as validated prevention approaches
  • Participate in diabetes committees and meetings in the communities, region and with outside agencies and government
  • Work collaboratively with other SERDC health programs and staff on joint initiatives and events


  • Professional designation as a Nurse, Dietitian, Athletic Therapist or Physiotherapist, with current registration, and in good standing with governing body (i.e., College of Registered Nurses of Manitoba, etc.)
  • Demonstrated experience in diabetes prevention and healthy living education, preferably in First Nation communities
  • Knowledge of the Diabetes Canada Clinical Practice Guidelines
  • Knowledge of scope and practice issues, liability, and standards of nursing/dietician/allied health profession
  • A strong background in program management which includes assessing, planning, implementation, and evaluation, but not limited too
  • Understanding of First Nations health care
  • Capable of taking initiative and following through to continuously improve internal processes when identified.
  • Must demonstrate reliability, responsibility, and dependability, in fulfilling obligations in an effective, efficient, and timely manner
  • Demonstrated group facilitation abilities and strong analytical skills.
  • Commitment to interdisciplinary teamwork
  • Communicate effectively with Federal, Provincial, Regional Health Authorities and other partners
  • Knowledge of other health organizations within Manitoba
  • Ability to adhere to all SERDC policies
  • Current CPR certificate
  • Certified Diabetes Educator certificate, an asset
  • Ability to speak Ojibwe, an asset

Conditions of Employment

  • Must produce:
  • a satisfactory criminal record check (including Vulnerable Sector Search).
  • a satisfactory Child Abuse Registry Check;
  • a satisfactory Adult Abuse Registry Check.
  • Valid Class 5 driver’s license, reliable personal vehicle, business use automobile insurance, including liability insurance coverage for up to $2,000,000.
  • Must be able to travel on short notice.
  • Capability to lift at least 50lbs and travel - by all forms of transportation, to remote and northern First Nation communities in MB including fly-in communities and out-of-province travel for up to 5 day(s) at times.
  • Overtime may be required
